Many teams collapse “witness” into a single person who both watches and approves. That shortcut feels efficient until a dispute appears and nobody can say who held which responsibility.

Observer vs authorizer

Observers confirm that steps happened as written. Authorizers assert that policy allows the next action. When one human does both, fatigue turns dual control into theater.

Scripts beat charisma

Give each role a short card: what they must say aloud, what they must initial, and what they must never touch. Charismatic leads tend to improvise; cards keep the room honest.

Remote witnesses

If a witness joins over video, document the channel controls separately from the wallet controls. A secure device in an insecure conference call is still a weak ceremony.
