“Cable found connected at 14:12; ceremony paused; isolation re-run by witness B” is useful. “Operator was careless” is not an audit note; it is a performance review leaking into the pack.

Link anomalies to gates

Tag each note with the workflow gate where it occurred—intake, isolate, transfer, or pack. Patterns across quarters become visible only when tags stay consistent.

Retention without clutter

Keep raw photos of anomalies for the retention window your policy demands, then summarize. The summary should still stand alone if the photo store is unavailable.
